Electroconvulsive therapy in a patient with resistant paranoid schizophrenia treated with clozapine

Introduction
One of the usual indications for Electroconvulsive Therapy (ECT) is Paranoid Schizophrenia (PS), being performed usually in cases resistant to antipsychotics.
ObjectivesTo present a clinical case of a patient with antipsychotic-resistant PS, including Clozapine, who received ECT.
MethodsWe present the case of a 47-year-old patient with an 8-years diagnosis of PS. He presented visual, auditory, and kinesthetic hallucinations, delusions, and thought insertion and diffusion phenomena that impeded concentration. He had received treatment with different antipsychotics (including Clozapine), without achieving remission of symptoms. He also presented significant adverse effects such as hypersalivation and extrapyramidal symptoms. Due to the poor response and the adverse effects that limited the dose increase, it was decided to start ECT.
ResultsThe patient received a total of 9 sessions, presenting a significant reduction in symptoms since the 5th session (disappearance of the sensory-perceptual alterations and thought disturbances). As side effects, the patient presented amnesia of the moments prior to applying the therapy, which subsequently resolved. The patient continued to present concentration difficulties, although after ECT he denied the presence of thought insertion or diffusion phenomena to which he previously attributed the cause of these difficulties.
ConclusionsAlthough less responsive than in other indications, ECT combined with antipsychotic drugs has been proven to be more effective than monotherapy (regardless of whether it’s Clozapine or another). This lower response could be due to the use of ECT in the most resistant cases, since it has been demostrated that in more acute cases a faster improvement occurs when the two treatments are combined.

Introduction
The association between cannabis and psychosis makes crucial the intervention on cannabis use disorder at first episodes of psychosis (FEP), especially among young population. In this group of patients, the harmful potential of cannabis is more evident by its influence on neurodevelopment. However, the nature of the association cannabis-psychosis is not clearly described. It seems to represent a mediating factor for an increased risk of psychosis in healthy and high-risk populations, determining an earlier age of onset and worsening long term outcome.
ObjectivesTo assess the impact of cannabis in terms of functional and clinical prognosis in patients recruited after a FEP.
Material and methodsPAFIP is an early intervention program for early stages of psychosis. One hundred and sixty-three were included, followed-up at regular intervals of six months for three years with administration of clinical and functional scales (BPRS, SAPS, SANS, CDRS, GAF and Drake). Patients were divided into three groups: (1) those non-users neither before the onset nor during follow-up (nn) PEP, (2) consumers before the FEP and during follow-up (ss) and (3) consumers before the FEP that gave up consumption during follow-up (sn).
ResultsNo statistically significant differences were observed in terms of functionality at three-year follow-up endpoint but a trend to a better-preserved functionality in the sn group. The sn group presented lower scores in scales for positive symptoms with respect to the comparison groups.
ConclusionsThe interruption in cannabis use may have a beneficial effect on short-term clinical prognosis and functionality on long term.

The Chilean species traditionally assigned to the genera Chlamys Röding, 1798 or Zygochlamys Ihering, 1907 are now placed in two new endemic South American taxa: Dietotenhosen n. gen. (middle Miocene–early middle Pliocene), to include the southeastern Pacific Ocean species D. hupeanus (Philippi, 1887) n. comb. and D. remondi (Philippi, 1887) n. comb., and Ckaraosippur n. gen. (earliest middle Miocene–Pliocene), for C. calderensis (Möricke, 1896) n. comb. (Chile) and C. camachoi n. sp. (Argentina). Both genera are the youngest survivors of the tribe Chlamydini in southern South America. None of them is related to the circumpolar genus Psychrochlamys Jonkers, 2003, and the previous proposal of the dispersal through the Antarctic Circumpolar Current for the species included herein in Dietotenhosen is rejected.

Zygochlamys Ihering, 1907 is revised and three new genera of tribe Chlamydini are named: the monospecific early Miocene genera Pixiechlamys new genus (type species: Pecten quemadensis Ihering, 1897) and Chokekenia n. gen. (type species: Zygochlamys nicolasi Morra, 1985), and the late Miocene–early Pliocene Moirechlamys n. gen., containing Pecten actinodes Sowerby, 1846 (type species) and Chlamys aurorae Feruglio, 1954. Zygochlamys is restricted to include Z. geminata (Sowerby, 1846) (type species), Z. jorgensis Ihering, 1907, and Z. sebastiani Morra, 1985. The present analysis increases the biostratigraphic usefulness of the group and improves taxonomic knowledge of the Neogene molluscan assemblages defined previously for Patagonia. Zygochlamys geminata is confined to the latest Oligocene–early Miocene interval of the Austral Basin of Patagonia (Argentina) and to the early Miocene of Chile, Z. jorgensis is restricted to the early to middle Miocene of the Golfo San Jorge and northern Austral Basins, and Moirechlamys n. gen., the most widespread genus, occurs in the late Miocene–early Pliocene of the Austral, Golfo San Jorge, Valdés, and Colorado basins. All these genera are endemic to southern South America; Zygochlamys is not related to other circumpolar genera such as Austrochlamys Jonkers, 2003 or Psychrochlamys Jonkers, 2003, rejecting its dispersal in the Antarctic Circumpolar Current, as has been proposed previously.

A very serious effort to modernize irrigation systems is being made in Spain, to reduce water and energy losses in an environmentally sustainable way. This is expensive and it is important that the new irrigation systems work properly over a long period. The systems have been designed taking into account historical evapotranspiration (ET) averages during the months of maximum demand, as well as the crop-specific ET values (Kc coefficients) of typical crops. However, the increase in ET rates due to global warming could mean that the capacity of these new and expensive irrigation systems to fulfil the crop water requirements may be exceeded in the near future. However, the expected increase in CO2 concentration could diminish crop transpiration rates for similar water demands from the atmosphere, thereby reducing irrigation requirements. A methodology was developed in order to estimate crop water requirements under climate change conditions. The reliability of a new irrigation system designed in Valladolid, Northern Spain was tested. The regionalized climate change scenarios for Valladolid, provided by the National Institute of Meteorology, were used for the periods 2011–40, 2041–70 and 2071–2100 and the A2 and B2 emission scenarios were considered using the ECHAM and coupled general circulation model (CGCM) global circulation models. A historical series of daily meteorological data for Valladolid was used to generate statistical ET distributions through the LARS-WG generator. Simulations considered each of the above periods, global circulation models (GCM) and emission scenarios. Furthermore, the Kc of the typical irrigated crops of the zone (maize, potato and sugar beet) were reduced for each period, GCM and emission scenario according to the relationships between CO2 concentrations and transpiration obtained by Kruijt et al. (2008). The results indicated that, on average, historical ET rates provide a sufficiently robust indicator to enable estimations of the crop ET in the future, particularly considering the CO2 effect in reducing crop transpiration. However, ET variability is significantly increased after 2040, especially for the A2 emission scenario. The results show that ET variability rather than global increase is the most serious risk that current irrigation systems must face in the near future in Northern Spain, as consequence of climate change. Such variability should be included in irrigation designs.
